In order to register as a patient at Weston House Surgery and Hatherley Branch Surgery, please visit in person, bringing with you photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it.

Alternatively, to speed up the process, please complete the forms below and bring with you to the surgery, along with your photographic proof of ID (e.g. passport or driving licence) and a utility bill or similar with your address on it.

Please note that the registration process can take up to 5 days.

We will ask you to complete our registration form, PDF copies of which can be found here for those aged under 16 and here for adults (those aged over 16) and here for babies (new born children)

Please also fill out the GMS1 Form (PDF) to register.

To register a new patient you will need to live within our practice boundary.
